Essentially a continuum of care system provides a comprehensive range of health services, so that the care can evolve with the patient over time.

Evidence-based guidelines, also called clinical practice guidelines, “are systematically developed statements to assist practitioner and patient decisions about appropriate health care for specific clinical circumstances” (Institute of Medicine, 1990).

However, most of the older adults participating in studies are interested in using a patient portal, regardless of health literacy level, previous patient portal adoption, or experience navigating health information on the web.

We understand that no one size fits all. We strive to tailor strategies to meet the needs of older adults (as well as their informal caregivers) by exploring alternative workflows that integrate both patient portal information into face-to-face contact with health care providers.
